Training Overview Description

Person-Centered Thinking, Planning & Services is a distance delivered course tailored to the needs of Person-Centered Planning Facilitators and families/staff who participate in planning activities for individuals who experience disabilities.

This class will teach person-centered thinking, planning, and service delivery approaches, focusing on Personal Futures Planning, MAPS and PATHS for people who experience disabilities.  The course will provide up to 20 hours of instruction and practice.
Participants will be expected to complete two person centered plans that will become part of their class portfolio.  Both plans will give real-life experience in using the tools and strategies taught in this class.  We will also explore Wrap Around approaches to service delivery, the importance of developing a collaborative team, solving problems when facilitating a person centered plan, and connecting the Person Centered Planning process with more traditional service planning.

Upon completion of the class and successfully completing all course assignments, the participant will earn a certificate of completion through UAA, Center for Human Development which reflects community learning hours invested in the course. This course is supported by Senior and Disability Services (SDS) with the State of Alaska, and may count toward ongoing certification for Care Coordinators.

Course Objectives:

By the end of this course participants will be able to:
1.     Demonstrate an understanding of Personal Futures Planning, MAPS and PATH person centered planning tools and processes.
2.     Demonstrate an understanding of the similarities, differences and applications of each tool.
3.     Develop a plan that focuses on an individual’s strengths, capacities, desires, and dreams and promotes community inclusion.
4.     Demonstrate basic process and graphic facilitation skills.
5.     Demonstrate an understanding of the importance of a collaborative team as part of the planning process.

Person-Centered Thinking, Planning & Services
A distance delivered course to teach and certify Person Centered Planning Facilitators offered by UAA Center for Human Development.

In order to take this course, PARTICIPANTS MUST HAVE:
- A current functioning computer with a webcam (required)
- A headset with a microphone (preferred) or computer speakers/microphone
- Access to high speed internet connection

The course will provide 15-20 hours of instruction & practice. Participants will complete two person-centered plans and must meet specific criteria to earn a Certificate of Completion.

This course is supported by SDS, and may count toward ongoing certification for Care Coordinators.

Cost: $100.00 covers materials, fees, and instruction.

The class will meet on Wednesdays from 9:00 AM – 1:00 PM January 17 through February 7, 2018. An optional open consultation session will be available February 14, 2018 from 9:00 AM - 11:00 AM with Final Presentations on February 21, 2018 from 9:00 AM - 1:00 PM.

This course is supported by SDS, and may count toward ongoing certification for Care Coordinators.
Cost covers materials, fees, and instruction. Participants will be expected to have access to a computer with a webcam (required), a headset with a microphone (preferred) or computer speakers/microphone, and a high speed internet connection.

Participants will be required to identify individuals within their current networks to complete the Person-Centered Planning Activities with.
Please note that you are expected to attend all class sessions unless prior arrangements have been made with the instructor.

Webinar Information: This training will be delivered as a webinar using Zoom. For those who complete registration and payment, the connection information will be emailed--to the email address in your user profile--the business day before the training.
