This class will teach person-centered thinking, planning, and service delivery approaches, focusing on Personal Futures Planning, MAPS and PATHS. The course will provide up to 20 hours of instruction and practice where participants will be expected to complete two person centered plans that will become part of their class portfolio. This course is supported by Senior and Disability Services (SDS) with the State of Alaska, and may count toward ongoing certification for Care Coordinators.
Person-Centered Thinking, Planning & Services
A distance delivered course to teach and certify Person Centered Planning Facilitators offered by UAA Center for Human Development.
In order to take this course, PARTICIPANTS MUST HAVE:
- A current functioning computer with a webcam (required)
- A headset with a microphone (preferred) or computer speakers/microphone
- Access to high speed internet connection
The course will provide 15-20 hours of instruction & practice. Participants will complete two person-centered plans and must meet specific criteria to earn a Certificate of Completion.
This course is supported by SDS, and may count toward ongoing certification for Care Coordinators.
Cost: $100.00 covers materials, fees, and instruction.
The class will meet on Wednesdays from 9:00 AM – 1:00 PM January 17 through February 7, 2018. An optional open consultation session will be available February 14, 2018 from 9:00 AM - 11:00 AM with Final Presentations on February 21, 2018 from 9:00 AM - 1:00 PM.
